RELEASE AND DISCLAIMER FORM

RELEASE AND WAIVER OF LIABILITY

I, in consideration of and as a condition of acceptance of my entry into Forever Young Lifestyle Co. Ltd, hereby release, waive, discharge, and hold harmless Forever Young Lifestyle Co. Ltd company, its officers, agents, employees and its trainers, instructors and assistants from any and all claims, liability, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, injury that may be sustained while using the services/products/equipment/facilities of MRC.

I am fully aware of the risks connected with Forever Young Lifestyle Co. Ltd services, including the risk of personal injury.

I assume full responsibility for any risk of loss, property damage or personal injury that may be sustained. It is my express intent that this Release and Waiver of Liability shall bind the members of my family and my heirs, assignees and personal representatives.

This Release and Waiver of Liability shall stand as long as I use the services and/or the facilities at Forever Young Lifestyle Co.Ltd now and in the future.

IMAGE RELEASE

I also hereby indefinitely grant and authorize Forever Young Lifestyle Co. Ltd the right to edit, alter, copy, any and make use of all photos and or videos taken of me to be used in and/or for promotional materials of any kind without payment or consideration. In addition, I waive any right to royalties or other compensation arising or related to the use of the photos or videos. I hereby release rights to all claims, demands, and causes to action which I have or may have by reason of this authorization.

NOTES

You are free to end your training at any time during any session by simply advising Forever Young Lifestyle Co. Ltd instructor who is always present with you to explain all exercises, guide and assist you throughout your session.

Any change in your medical condition between sessions should be mentioned to your Forever Young Lifestyle Co. Ltd instructor.

Forever Young Lifestyle Co. Ltd is not responsible for any of your personal belongings left behind or lost.

CLIENT SIGNATURE

By signing this consent and release, I acknowledge and represent that I have read, fully understand and agree on the above Release and Waiver Form. I am at least twenty(20) years of age and fully competent; sign this of my own free will. Furthermore, I agree that I will comply with all instructions on the use of the facilities, equipment and/or exercises and that I am using Forever Young Lifestyle Co. Ltd services at my own risk.
